Grip Strength and Longevity: Why It Matters and How to Train It

Grip strength often gets treated as a side effect of other training — something that improves because you are doing pull-ups or carrying things, not something worth thinking about directly.
But researchers have studied grip strength for decades, because it turns out to be a remarkably useful window into overall health. And the data is consistent enough that it is worth understanding what it shows and what it means for how you train.
What the Research Says
The PURE study tracked 139,691 adults across 17 countries. For every 5-kilogram reduction in grip strength, researchers found:
- 16% higher all-cause mortality risk
- 17% higher cardiovascular death risk
- 7% higher heart attack risk
- 9% higher stroke risk
Grip strength proved a stronger mortality predictor than systolic blood pressure in that dataset.
A UK Biobank study of over 500,000 participants found similar patterns. An umbrella review confirmed associations with lower all-cause mortality, reduced cardiovascular mortality, and decreased disability risk. A 44-year prospective study showed that midlife grip strength predicted survival into advanced age, including reaching 100 years old.
Does Grip Strength Directly Make You Live Longer?
These studies show association, not causation. Weak grip may reflect underlying issues: muscle loss, physical inactivity, chronic illness, poor nutrition, or reduced nervous system function.
Grip tests are useful to researchers precisely because they efficiently capture many health factors simultaneously — using an inexpensive hand dynamometer. They do not prove that forearm training controls lifespan. What they indicate is whether someone is maintaining muscular strength and physical function overall.

Why Grip Reflects More Than Hand Strength
Producing a strong grip involves the hand, wrist, and forearm — but the measurement also reflects the entire body's capacity. Force generation depends on muscle quality, nervous system function, joint health, energy availability, and training status.
Healthcare professionals use grip strength when assessing sarcopenia and frailty in older adults, because it indicates overall muscular function decline. Research links every 5-kilogram reduction in strength to limitations in eating, walking, bathing, dressing, transferring, and toileting.
This connects directly to practical longevity: being able to carry groceries, lift luggage, get up from the floor, support your own bodyweight, and remain independent. Those are the outcomes that matter.
Why Grip Matters in GST
Nearly every pulling and hanging movement at MOVE involves grip:
- Dead hangs and active hangs
- Chin-ups and pull-ups
- Ring rows and ring dips
- Muscle-up progressions
- Front and back lever training
- Hanging core exercises
- Loaded carries and barbell work
Athletes often have enough back strength for more pull-ups, but hands give out first. Ring training adds complexity because you must hold the rings while controlling movement — coordinating grip, shoulder stability, pulling strength, and body control simultaneously.

How to Train Grip Strength
Most people do not need complicated grip routines. A balanced approach covers several gripping types: hanging grip, support grip, crushing grip, and pinch grip.
1. Start With Hanging
Dead hangs teach bodyweight support from the hands. Active hangs add shoulder blade control as preparation for pulling.
Beginner starting point: two to four sets of ten to thirty seconds, with adequate rest. If full bodyweight is too much, keep feet on the floor or use a lower bar. Progress gradually — do not force hangs when hands are slipping or shoulders are in pain.
2. Train Pull-Ups, Chin-Ups, and Rows
Pulling exercises build grip simultaneously with back, arms, shoulders, and core — making them far more valuable than isolated hand work.
Beginners progress through ring rows, incline rows, assisted chin-ups, or controlled negatives. More experienced athletes work strict pull-ups, weighted variations, towel grips, or uneven pulling. Use progressions that allow controlled movement rather than treating every set as a grip test.
3. Add Loaded Carries
Farmer carries are simple and effective. Pick up weights in each hand, stand tall, and walk while maintaining control.
A practical starting range: two to four sets of twenty to forty seconds, with loads you can sustain without postural loss. Suitcase carries with uneven loading increase trunk demands while training the working hand.
4. Use Static Holds
Hold barbells, trap bars, or dumbbells for set durations while maintaining shoulder control. Add these after deadlifts or rows. Start with short, controlled sets rather than holding to complete failure — repeated maximal holds create unnecessary forearm and elbow fatigue.
5. Train the Fingers and Thumb
Pinch-grip exercises strengthen fingers and thumbs differently from hanging or carrying — holding weight plates together, carrying blocks, or using specialised implements. These are useful for advanced athletes and climbers but optional for beginners, who already receive significant stimulus from pulling, hanging, and carrying.
6. Don't Ignore the Forearm Extensors
Most grip exercises involve closing the hand repeatedly. Including movements for the opposing side helps condition the tissue for repeated gripping:
- Wrist extension with light dumbbells
- Finger extensions with rubber bands
- Controlled wrist rotations
- Light reverse curls
Those with existing wrist, finger, or elbow pain should adjust rather than force grip work.
How Often Should You Train Grip?
Most people do not need separate daily grip workouts. Two or three weekly exposures are sufficient when grip work is integrated with pulling, carrying, and strength training.
A simple weekly structure:
- Hangs and pull-ups during upper-body sessions
- Farmer carries during full-body or lower-body sessions
- Light wrist and finger work as a finishing piece
Hands and forearms need recovery. If grip weakens session to session, elbows stay irritated, or forearm soreness starts interfering with other training, reduce volume. More is not automatically better — the goal is sufficient stimulus without compromising everything else.
Grip Training as Part of a Bigger Picture
Research from the British Journal of Sports Medicine found that muscle-strengthening activity associates with 10–20% lower all-cause mortality and cardiovascular disease risk, with the greatest benefit around 30–60 minutes per week.
The WHO recommends adults perform muscle-strengthening activities involving major muscle groups at least twice weekly.
Train grip alongside legs, hips, back, pushing, pulling, balance, mobility, and cardiovascular capacity. Grip strength is most useful when it improves as part of a stronger, more capable body overall.
